* Please note that if you intend on purchasing more than one plate, you will need to return to this form again after completing it successfully the first time.

Please enter your information requested below. Please complete ONE form for EACH vehicle for which an EMS Specialty plate is ordered. You may combine payment for multiple forms within the same household if printing and mailing in your requests.

Please send payment (no cash) for $35.00 for EACH specialty plate ordered and make payable to TEMSEA. Personalized specialty plates may be ordered by adding an additional $35.00 per personalized specialty plate for a total of $70.00.

License plates on your vehicle: If you have time on your current plate or if you renew your license plate when your EMS plate is ready, you will only pay a pro-rated amount. You will NOT have to pay full price for two tags.
